NEW DEADLINE FOR NOMINATIONS: FRIDAY, APRIL 13, 2012

Eligibility

  1. At least one of the following two criteria must be met by the nominated individual:
    1. Nominees must have made a remarkable research, policy, or advocacy contribution to the cause of reducing tobacco use in the United States through the use of tobacco industry documents, and/or;
    2. Nominees must have employed innovative, creative approaches to the employment of tobacco industry documents that result in an improvement in the health or public awareness of the United States or one of its states or localities.
  2. Nominees must not have any affiliation with the tobacco industry.
  3. Nominees for the Christine O. Gregoire Youth/Young Adult Award must be 24 years of age or younger.

Any individual meeting the above eligibility criteria and the qualifications set forth in the following guidelines may be nominated, including but not limited to educators, social workers, documents researchers, public health practitioners, legal researchers, grassroots organizers, activists, lawyers, academic and scientific researchers, government officials, physicians, and staff of community-based organizations (CBOs).

Guidelines

  1. Nominations may be made by individuals such as colleagues, peers, coworkers, instructors/professors, governments, or community-based organizations that are qualified to make such a nomination because of their familiarity with the nominee’s work and contribution.
  2. Individuals may not nominate more than one person for any one award; however, they may submit a separate nomination for each of the two categories.
  3. Nominations should be made according to the application outline contained in the Call for Nominations and must be accompanied by a minimum of two letters of support. One of the letters of support must be from someone other than the direct supervisor of the nominee or any employee of the organization/company for which the nominee is a member of the board of directors.
  4. The nominator must answer all questions posed in the online application.
  5. The only method of submission is through the Legacy Awards website.
  6. Completed applications will be acknowledged electronically.
  7. Incomplete applications will not be considered.
  8. Applications received after the NEW deadline (Friday, April 13, 2012, midnight Eastern time) will not be considered and will not be returned.
  9. Members of the awards committee may not nominate potential recipients.
  10. Employees and directors of Legacy and their relatives are not eligible for either award.

Terms of Award

The selection of the Legacy Tobacco Industry Documents Award recipients will be made by a distinguished awards committee comprised of nationally recognized individuals in the tobacco control and related fields. Each recipient will receive a cash award (given to the recipient or an institution or organization upon the recipient’s request), a plaque, name inscription at the Center for Tobacco Control Education and Research at the University of California, San Francisco, and a mention in a Legacy press release promoting the awards ceremony. Award recipients will be expected to attend the awards ceremony at the National Conference on Tobacco or Health to be held August 15-17, 2012 in Kansas City, MO.

Questions

Please answer each question. Use 250 words or less to answer each question.

  1. What is the nominee’s involvement with the tobacco control field? How long has he or she been active in the field?
  2. How has the nominee used the tobacco industry documents in his or her work?
  3. What makes this applicant’s use of tobacco industry documents unique and innovative for policy, advocacy, or research?
  4. What is the impact of the nominee’s work with tobacco industry documents on public health or public policy initiatives? Describe changes that have resulted from his or her work.
  5. Describe “lessons learned” from the nominee’s work that are applicable to other potential uses of documents in research, policy or advocacy.

Additional question for Christine O. Gregoire Youth/Young Adult Award:

  • How has this work raised awareness or demonstrated leadership among the nominee’s peer group?


Two Letters of Support

A minimum of two letters of support must be included in the application. In addition, two other supporting documents may be submitted (i.e. research articles, news articles, educational materials, etc). (All documents should be uploaded electronically in Word or pdf format). One of the letters of support must be from someone other than the direct supervisor/boss of the nominee or any employee of the organization/company for which the nominee is a member of the board of directors. Organization letters should be signed by the leader(s) of the organization. Letter writers are encouraged to include relevant quotes, anecdotes, and experiences that help describe the impact of the nominee's use of tobacco documents in research, policy, or advocacy.

Timeline

  • Applications due: Friday, April 13, 2012, midnight Eastern time
  • Winners notified: May 2012
  • Awards ceremony: to be held during the 2012 National Conference on Tobacco or Health, August 15-17, 2012 in Kansas City, MO.

In the event of a submission error, this will preserve your answers so that you may simply re-paste them into each field instead of retyping them.
